Just concentrate on keeping your head still throughout the whole swing. Also ease up some, you may be swinging out of your shoes.

I bet you're trying to lift the ball...hit it forward, not up. Try to keep your shoulders level and hit down and through.

Your head and right shoulder fall down during the down swing (perhaps even before, when you start the back swing).

I had a similar problem. Irons went fat or crashed into the soil but drives, since the tee is tall enough, went right to the sky.

I only could fix the problem using a mirror where I could see exactly when my head fell down. My backswing had kind of two phases, and I only did that in the second one, so when I started backswinging everything looked ok, so I was getting crazy.

Once you have swung in front of a mirror and spotted the place and time of the fall, I recommend the following swing thought:

Be as arrogant as a king during backswing, downswing and followthrough. Keep your head and dignity high.

Simple corrections. You are more then likely setting up to low, (shoulders) when addessing the ball. And you are dipping the left shoulder downward when you start your backswing. If a person sets up too low and drop the shoulder, they do not have enough room to swing though on the downswing, therefore, they will feel that unconsiously, and raise up to create the needed room. By not turning the shoulder, dipping instead of rotating them only adds to the problem. Cure: Stand taller and feel like you are streching upward with your shoulder while feeling down with the rest of the body/arms. Start the backswing by turning the left shoulder out and around and have the feeling that you head is tilted slightly to the right. You will then feel like your are turning the left side around the spine. With the head in that position you can't come over the top and it will promote more of an inside back move which in turn will promote a draw and more distance. The golf swing is two turns, turn back and then turn back through! That means around not down!
