Your head and right shoulder fall down during the down swing (perhaps even before, when you start the back swing).
I had a similar problem. Irons went fat or crashed into the soil but drives, since the tee is tall enough, went right to the sky.
I only could fix the problem using a mirror where I could see exactly when my head fell down. My backswing had kind of two phases, and I only did that in the second one, so when I started backswinging everything looked ok, so I was getting crazy.
Once you have swung in front of a mirror and spotted the place and time of the fall, I recommend the following swing thought:
Be as arrogant as a king during backswing, downswing and followthrough. Keep your head and
dignity high.
