Club shaft is across the line and club face open at the top from the rear shot. Take a look at the Jeff Ritter video below for problems and fixes for that. From your straight on shot it seems to me that you don't fold your right elbow soon enough. In other words you keep your right arm straight well into the back swing only folding after you lift your arms. That gets your elbow too far from your body resulting in the across-the-line position at the top. When I look at your address, I wish the shot was more straight on, but it looks like to me you are leaning the shaft back. That is not a good address position for irons especially the more lofted clubs. For must clubs the shaft should lean forward with the driver being the exception.
I was looking at your comments. You have the right perception that you are going off plane at the top. Have you had any lessons? There are many things on which you need to work and lessons may shorten the learning curve for you.
