rocking back on my heels...

driver/3wood I am ok.
But my irons I seem to end up rocking back on my heels, and leaning back/dipping my my right shoulder. or if that does not happen, I hit the ground first. Am I standing too close to the ball?

Well being a student of Hogan for years, the weight should be a little more should be on your heals from the start.
